Steps to Complete the Feedback Form

1. Open Google Chrome and enter this link:Website Link

2. Enter the username and password exactly as provided.

3. You will see the names of the teachers for the second semester. Read and score the 12 questions one by one. Ensure that the scores are not all 1s or all 5s, as this will prevent you from proceeding to the next step.

4. Do not leave any fields blank. Once you finish answering the questions for the first teacher and fill in all fields, click on \\'next\\' to access the feedback form for another teacher. Follow the same procedure until you reach the last teacher, at which point the system will close automatically.

5. All learners are required to participate; otherwise, they will be held accountable.

6. Contact the department if you encounter any issues.
